How do I build a custom trap from these tables?

Roll once on trap triggers ("stepping on the third flagstone"), once on trap effects ("a cloud of paralyzing gas"), and you have a unique trap. Add a dungeon trap description for extra flavor.

Do the traps include difficulty or damage numbers?

No. The entries are descriptions. Set difficulty, damage, saves, and other rules using the game system and table style you use.

Are wilderness hazards different from traps?

Yes. Wilderness hazards are natural or supernatural environmental dangers - quicksand, poisonous flora, rockslides, cursed groves - rather than constructed mechanisms.
